Massivly improves on the standard magento checkout and does it in style with quick and trouble free installation and an instant boost in sales. Anyone who has set up a funnel with google analytics will know how many people drop out part way through the old and long checkout process. This extension instantly increased the conversion rate for us.
It also adds some nifty new features like newsletter sign up, terms and conditions, auto account creation and more. Support were even good enough to help me make it work with a customised postcode address lookup module.

I run a marketing agency and we strongly recommend that all Magento clients we take on implement this extension to maximize conversion rate and average order value. The higher our clients convert traffic the more revenue and ROI they see from their investment with us and although the prices can be a little high, the customer service that I have received from one step checkout is second to none. The default shopping cart for Magento adds several steps and is not very user friendly. In an effort to make our site as user friendly as possible this is a must have extension.
Recently I upgraded to the latest version and the cart wasn't working properly. I submitted a ticket late Friday afternoon. I wasn't expecting a response until Monday but they responded right away and fixed it within hours. We have a Magento installation that has 4 different websites and they are all running OSC. All you need to do is have a different code for each domain and put that in the Admin area.
We also have merged the cart with the checkout to a single page and it works beautifully (our main site is www.desirablebody.co.uk - so come and look). Whilst OSC generally works out of the box, if you need to do some customisation it's pretty easy to find where it all needs to be done.
We have noticed quite a drop in Abandoned Baskets and also a drop in customers calling in with problems checking out - the standard One Page can confuse people.
All in all - an awesome addition to a Magento site. Any decent Magento setup should have this as a pre-requisite.Posted 5/5/2011 Functions as described Developer Support Usefulness -

In 5 minutes i have done a complete restyle of my checkout process. Before this extension my customers needs to pass trought 5 different steps. Now in a single page, with the autocomplete on address fields utility, and several others widget, like gift wraps, coupon and so on. i encounter an issue, but with the support online I solved in ONE session. Conversion booster, great functionalities, I bought my 1st license a while back. A second one now, and I even got the free update to the latest version packed with even more awesome features all manageable from the admin!
Great customer support, I asked for a custom invoice done within hours, professionalism all the way PLEASE can somebody give Jone and his team an Oscar for best Magento Extension ever!

It's expensive for small shops, but it's worth every penny.
Easy to install, works with other payment and shipping extensions and you can select default payment and shipping method.
Customer service is also great. Even though my support period ended, they didn't charge me.

I spent days/hours looking, and this one really is the cat's meow. I teetered back and forth trying to justify the expense - as this ones not a cheap upgrade - but after making the purchase, I can tell you, you definitely get what you pay for. This is a very customizable module from the admin backend, very well thought out, and makes such a big difference from the agonizing step by step by step default checkout that comes with magento. These guys put a lot of effort into this one and it shows.
The support was also excellent and I was up and running practically hours after I made the purchase! Check it out in action on www.installerparts.com

We purchased the OneStepCheckout extension with the understanding that there might be a few issues because of the nature of Magento and simply just because of all the customizations that we have in the store itself.
We installed this extension our-self and were very pleased with the just how simple it was to install. We tested it for processing Check/Money order orders and it worked flawlessly. When we finally went live the extension would not validate the credit card! For 2 days we pulled our hair out until finally contacting OneStepCheckout. Even though they were not obligated to look into our issue they did and fixed the issue for us. Previously they also went out of their way and worked with another company that we purchased a shipping Matrix from, the problem with this was is the shipping Matrix did nto tie into OneStepCheckout. To make a long story short they went over and beyond their call of duty and helped get OneStepCheckout configured to work with our Shipping Matrix Over-ride Extension.
I am glad we purchased this extension and did not go with a free one for not only the support, but this is definitely the best extension on the market for simplifying the checkout process and we have seen sales increase because of it.
